By signing below, I acknowledge and agree to the following regarding my windshield rock chip repair performed by Glass RX:

1. Crack Propagation Risk. Windshield rock chip repair is a cosmetic and structural stabilization service, not a guarantee against further damage. In some cases, a repaired chip can crack or spread across the windshield during or after the repair process, due to temperature changes, road vibration, impact, or the pre-existing condition of the glass. I understand that if this occurs, Glass RX is not liable or responsible for windshield replacement costs, and no refund will be issued for the repair service.

2. Cosmetic Results Vary. Rock chip repair improves the appearance and structural integrity of the damaged area, but results vary by chip type, size, age, and contamination. Some chips will become significantly less visible while others may remain partially visible after repair. Glass RX makes no guarantee as to the final cosmetic appearance of any repair.

3. Tinted Windshields. If a windshield has tint film applied, our technicians cannot apply direct heat to the glass during the repair process, as heat can damage the tint. This limitation may result in a less optimal repair outcome (in cure quality and/or appearance) compared to an untinted windshield. I understand and accept this limitation if my windshield is tinted.

4. General Acknowledgment. I confirm that I have read and understood the above terms, had the opportunity to ask questions, and voluntarily agree to proceed with the rock chip repair service under these conditions.
